How to Fill Out a Bill of Lading
Follow these four steps every time to stay compliant and protect your company from liability disputes.
Fill in shipper and carrier information
Enter the customer's full legal name, origin and destination addresses, and move date at the top of the form. Add your company name, USDOT number, and MC number. These identifiers are required by FMCSA and give the document legal standing.
Complete the household inventory
Walk through the home room by room before loading begins. List every item, its quantity, and note any pre-existing damage in the condition column. Both you and the customer should agree on the inventory before signing. This list is your evidence if a damage claim is filed.
Record weight, cubic footage, and pricing
Enter the estimated weight and cubic footage — or the actual weight if you're doing a weigh ticket. Reference the estimate number from your original quote. Mark whether the estimate is binding or non-binding, and record the total charges and payment method.
Collect signatures at pickup and delivery
Both parties sign the BOL at origin before the truck is loaded. At delivery, the customer signs again to confirm receipt of goods in agreed condition. Any exceptions (missing items, new damage) should be noted in the exception section before the customer signs at destination.
